This form must be used for the following modifications and mailed to the Board at BWCA, P.O. Box 1885, Duluth, GA 30096. Please send form before any hiring of contractors takes place to allow ample time for processing by the Board.

1)Contact Lisa Cooper 770-582-1658, email: lrcooper@bellsouth.net to reserve your date on the calendar.
2)Complete both the Facilities Use Agreement and Clubhouse Application for Reservation Agreement.
3) Write two checks to the Berkeley Walk Community Association. One for the $100.00 rental fee and one for the $100.00 deposit (which will be returned
upon a satisfactory inspection of the clubhouse).
4) Place both agreements and both checks in Lisa Cooper's mailbox prior to the rental date.
5) A key will be delivered to you prior to your rental date.
6) The key must be returned the day after rental.
7) The clubhouse will be inspected and your deposit will be returned.
